This will be South Warwickshire NHS Foundation Trust’s fourth annual integrated care event. The day will build upon information shared at previous events and demonstrate the results of the work implemented to date, which earned us Highly Commended for Quality and Productivity at the HSJ 2013 Awards.

Who should attend?

- Representatives from acute and community providers, CCGs, CSUs and local authorities who wish to integrate their emergency and/or elderly care
- GPs with a keen interest in this area
- Third parties e.g. voluntary sector who have an interest in supporting such integration work
- Previous attendees as we will build on information at provided at previous events

Will include areas such as Stakeholder Engagement, vision funding, governance, nursing home bed procurement and monitoring, metrics, lessons learned

Discharge to Assess (D2A) - operational view

Will include the day to day practicalities such as documentation, team structure, documentation used, lessons learned

The team will describe the driver for developing these initiatives, how it is managed on a daily basis, tools developed, lessons learned, and aspirations for the future.
